    These forms are to support your payroll service. Download the appropriate form, complete all required fields, and send to your service bureau or Customer Support team. Please use an adjustment form only if you cannot update the information yourself with your online payroll.

    • Waiver of Direct Deposit Prenote Requirements: Complete this form only if you have direct deposit services, you want to waive the prenote process requirement, and you agree to release Sage of liability.
    • Payroll Funding Account Authorization: Complete this form to change the bank account you use for funding payroll.
    • Employee Setup or Change Form: Complete this form to set up a new employee or to change existing employee information.
    • Employee Direct Deposit Form: Have your employees complete this form to set up or change their bank accounts for direct deposit payroll.
    • PTO Plan Form: Complete this form to set up a new paid time off (PTO) plan or change your existing plan.
    • Garnishment or Court Order Information Form: Complete this form to set up a new wage garnishment for your employee's paycheck.
    • Sage UPS Savings Form: Complete this form to enroll in the UPS Savings Program available for Sage customers.

    These forms are to request adjustments to your payroll data specifically for year end. Download the appropriate form, complete all required fields, and send to your service bureau or Customer Support team. Please use an adjustment form only if you cannot update the information yourself with your online payroll.
